Conwy Partnership for Health, Social Care and Well Being questionairre project. Aim was to get the local community involved in gaining information to help improve standards in public services.

The brief was to create an eye catching theme that was different to current literature to attract awareness in doctors surgerys and hospitals in North Wales.

The project included questionairres, posters, banners, post boxes and results boards.
